Ensure that the awarding body is on the approved list as provided in the link above.

The qualification should be at least at ESOL Entry Level 3 or higher in order to meet the English B1 required level of the CEFR.

Keep checking for dates for LIUK test on a daily bases, there maybe a cancellation, you may get lucky. You will need to have patience. 6 weeks of waiting time is nothing for a spouse visa.

Standard time is usually around 3 months and some people can easily be waiting around 6-12 months before they hear anything. Processing times will vary depending on demand.

You cannot change a visit visa into a spouse visa. As Rayking as mentioned, you will have to return to the US and then apply for a spouse visa from there. If you have satisfied both English B1 and LIUK test for ILR, then these can be used again for Naturalisation.

I would submit the evidences again in addition to ticking the boxes. Remember, you want to make the case worker's task as easy as possible not to allow any complications.

Has your wife taken the Life in the UK test yet?

To be honest, if she is struggling with the English test, then the LIUK test is going to be even more difficult. You may consider early preparation for LIUK long before you are to apply for ILR.

I would assume the head office for the Home Office is based in Liverpool (their passport office certainly is) so the transaction would be showing as Liverpool.
