Not entirely sure but have you been granted a 5 year leave based on that CoS from 31st March 2011. I guess not as 5 years wasn't an option at that time. If not you would have used a further CoS to extend your leave. In any case while you meet the salary threshold, you shouldn't bother. Good Luck

The work start date on your CoS is indicative. This helps the case worker decide on the length of your leave. One cannot be expected to start on the exact date mentioned on the CoS. Unless it is way out, you should be fine.

You are not accumulating any time towards Indefinite Leave (5 year Tier 2 route) on your current visa. The time spent on Tier 2 ICT visa doesn't count in the 5 year eligibility if you are thinking that route. If you are aiming for 10 year long residence your stay from 2013 would be counted provided...

No you should be ok. Cool off applies to your ability to re-apply as Tier 2 main applicant not as a PBS dependent.
