Question H should be left blank...Do not write anything by hand on the form apart from your signature.

In regards to the surrender certificate you can submit self-attested photocopies of the Certificate along with your application the original surrender certificate would not be required.

I suggest apply online and try to apply in person, take with you your mother's previous and current marraige certificates, did your mother have her cancelled Indian passport with new surname or previous name that mathces to yours?

OP - each application is dealt with separately and some takes longer to process, as you are naturalised as a British Citizen you will get your passports, its a right of a British citizen, just hang in there!
