NHS not allowed to counter sign anymore. You ought to know someone that could sign. If you know a store manager of any limited company they can sign for you and its acceptable. Like Asda, Sainsbury's, pizza hut etc. Also accountants, solicitors or minister of a recognised religion can do it... Go t...

Thanks. Could be this issue, they didn't mention any reason for interview call. I think that could be the reason. I applied in Jan and didn't want to complicate the things with permanent+ self employment income. I am self-employed now. However earnings I showed for this extension is from permanent job with period from Oct2013 till Jul 2014. Are you employed or self employed? If you're self employed then I think they'll ask ques about your self employment.
