No issues here. My payslips do not mention my middle name but my bank statements do. I got ILR last week w/o issues.

Phew! finally got my ILR 1. Category. Tier 1 (General) 2. Number of dependents. 2 3. Total cost. I booked the slot on 02 Mar 2015 and paid the old fees but went for appointment on 16 April 2015. I did not fill in the payment sheet. New fees were not asked as my appt was booked before the fee change....

I am going for my ILR in 2 days. I have not ticked this.

My understanding of the form is that after subsection E1 of section 9A we need to move to Section 10.

Section C under Section 9B is only for Tier 1 Entrepreneur.

Correct, it is not there for Tier 1 (G) anymore but one can use any other category (like Tier 2 or Tier 1 E) and claims points for degree + english language if it is awarded, irrespective of applicant's current category.

If your payslips are not original then you have to do one of the following: * Take a print out of your e-payslips and get it certified by your employer. Each and every page of the payslip needs to be certified. Certified means stamped, signed and dated. I used this format for my Tier 1 (G) extension...

Correct, you have to select Tier 1 General. Once you go along pressing next, you will see on some screen stating that the application for Indefinite leave to remain (Set O). I find this question, SET (O) 7.11, a bit odd... so if someone enters the UK after 2.5 months of visa start date: they are eligible to apply for ILR within 28 days of the visa expiry (since they came in within 90 days) BUT their stay in the UK, at the time of applying, will be 4 years and (maybe) 9 ...

Your 5 years are completed on 10th May 2015, so 28 days prior to this date. Whether you are paid or not for some months during your employment are irrelevant as long as you get enough points on PBS calculator. Thanks tier1o.
