This is a new requirment by the Home office, they never asked for this info before, as this form has only been out 3 weeks it is to early to see how they are going to view future PR applications.
The simple answer is we just do not know, as you are still with your EEA spouse, i do not believe it would be automaticlly refused, but the new form is designed to look at the relationship over the 5 year period. it is early days yet
Sorry i do not know, but read the section on ROR, there are some important changes it looks as if getting ROR is going to be harder, there have been a few posts, from people being called to Liverpool for interview.
The forms have changed eea 4 is now called EEA(PR) ROR SS etc etc are all on this form it is 126 pages long, just fill in the sections that apply, this form also replaces EEA3 forms, while the EEA1 AND EEA 2 forms are now called EEA (FM)
Have a look at the topic, in the EEA forum(the new PR FORM), sorry could not get a link to work, the new PR form is 192 pages long and goes into quite a lot of detail on benifits recieved, so i doubt we can know exactly WHAT THE OUTCOME WILL BE.
You should write to them(post) and send it recorded, use the references you would have received when you got the coa and letter confirming your application, you should do it as soon as possible incase they contact the old employer
The new form effective 31 Jan EEA(PR) is 121 pages long suggest you look at it as it seems they want a lot more proof of residence 3 or 4 items for each year and from 5 or 6 different sources, For the first 3 months you do not need to exercise treaty rights, so I very much doubt that 1 month nearly ...
