If she is doing the IELTS SELT, then HO would get the results electronically. You only need the SELT number for the form and you can write a note explaining you are waiting for the test result sheet/certificate. You dont need to mention the previous refusal. Consider it as new application and explain anything you attatched one by one. Plus mention your mums previous visit to italy and canada.
