To distinguish between forenames and surname, you can put the former in small letters and the latter in capitals e.g. John Joshua SMITH.

John Joshua = Forenames
SMITH = Surname

Many Thanks JohnnyZee. I did applied with my current British passport and deed poll that's it. I didn't include cover letter or uncancelled passport as checked with some one on customer services and they said it should be sufficient.
