settlement from Tier 1 Application form for dependant

Hi,
I have a question regarding my wife's visa. I am on Tier 1 (General) and I am eligible for ILR in Sept 2015. We are applying together for ILR visa.
Is there only one application form for both of us SET (O) or there are separate application forms for main applicant and for dependant?
