If you have completed the application online and paid the IHS through the same application then the IHS reference number would be mentioned on your checklist.

The documents which are mentioned in the checklist is all that is required.

Ideally, the IHS fee will be refunded by the home office if the application is unsuccessful. Not really sure what will happen in your case if you make the another payment. You can try making another application and in the page where you need to pay the IHS, you can mention the reference number you received in the email.

I remember seeing a text box to input the reference number if you have already paid the IHS fee. Just give it a try. I hope it works.

Yes. You can ask the sponsor to add a note.

I think the status will be changed to 'Used' after a decision is made by the home office. irrespective of the decision being positive or negative.

Yes. The sponsor note can be added anytime till the COS is active. Until the decision is made.
