Yes at premium service you can not vary your application but you can do by using the paper application or online via the new system.

Also, you mention that Upper Tribunal dismissed your appeal with the right to appeal. Don't understand this? After dismissing the appeal by upper tribunal what right of appeal you have given further.

According to my understanding when you applied for the visa and it was in time application, once your request for fee waiver rejected you have given 10 working days to pay the fee. You dont need to submit anything just filled ILR (LR) form. Life in the UK test + English language requirment. Also I advise you to book same day service. One of my friend just got his ILR on long residence after his extension was approve 3 months ago. Simple process. Wish u best of luck
