If you are on a work permit/PR in Canada and applying from there for Tier 2 I do not think you need to give a TB test certificate. I applied from Toronto and got it done without TB test. Dependent visa are in line with the Main applicants visa. If your visa gets over I am sure that hers will too. She cant continue to work without a visa.

No. You will not be able to apply for Tier 2 general without serving a one year cooling off period from your ICT visa end date or date of departure from UK, whichever is earlier.

Yes it is possible but only after serving a cooling off period of 12 months. If his spouse can get a Tier 2 General then he can switch to Tier 2 Dependent category inside UK.

You will need to get Tier 2 General for which you need to serve a cooling off period of 12 months outside UK. Alternate options are to become a Tier 2 dependent if your spouse (if you are married) gets sponsored on his/her own rights.

For changing job you will need to serve a cooling off period starting the day you left UK or the end date of your ICT visa whichever comes earlier. You will need to spend 12 months outside UK before coming back to UK for another job.
