My Advice is, Prepare a Good Resume mentioning your work Status for UK e.g. I put Work Authorization: UK (under HSMP) and INDIA. Normal Copy of Passport will also do... Notarized Copy is not required but as u hv to send Notarize Copy with ur application, u can mention "DoB Proof: Notarize Copy of Passport"

You Need not to Re-apply for HSMP. But you can apply for EC again.
