What you need to show is that you have been exercising the treaty rights for 5 years. If you have been working, you need to send p60 for all those relevant years. If you have been self-sufficient , you need Comprehensive insurance. Just ensure you have been exercising the treaty for plain 5 years. ...

The application for a document certifying permanent residence for EEA nationals takes 6-8 weeks in most cases, not 5 months.
