What to include in application and when to apply

Hi,
Q1. Could anyone tell me please what documents to include in 10 years long residency application. I have already done life in the UK.

Q2. I came to the UK on 01/09/2003. Can I apply on or after 05/08/2013 28 days before my 10 years completion.

Thanks in advance for your reply.
