integra check correct form??

HI,
there are two forms
i)nonsettlement form and other one
ii) HSMP
which one to fill?
nonsettletment form also has hsmp as a checkbox and this one asks for lot of details and past 2 years employers details.
whereas hsmp form asks only for last one year.
pls clarify.

Does the payslip need to have employers sign and seal?
I think it required only in the case of online payslips i.e. if you take a printout of that then employer need to endorse it, isn't it?
