From GOV:

had permanent residence status for the last 12 months if you’re a citizen of an EEA country - you need to provide a permanent residence document

12 months since when? Receiving it...or 12 month since you actually acquired you status and document is just to prove it.

Really confusing as I know many people who think that they need to wait 12 months. I know Ukrainian guy whom got his PR card on September 2016 (lived here since 2006) and was told that he can apply only if 12 months pass that is for him September 2017...really confusing. I was told by UKBA that as an EEA citizen who lived in the UK for over 10 years can apply for naturalisation only if 12 months passed since issue date on my PRDC (??). Thanks but that does not explain much..... So present address don't need to be put again in section 1.17 as is mentioned already in 1.16 .???? So in section 1.17 I put all apart from present one ..yes?

Silly question...but more I think the more I get confused. Had 2 address in past 5 years. So I put current address on page 4. Point 1.16 so in point 1.17 I put my previous only. No need to put current again...?

Just checked and it says Unique Home Office Reference number. And this is my very first documentation in the UK. WRS card as back that time as a EU citizen of A8 countries had to apply for Workers Registration Scheme (Work Permit) and this number appears there.

Noajthan. I was referring to this. So believe it should be ok as HR Manager is a person dealing with internal data and employment related issues. Just need to be sure as this is last section to complete before sending my application out. Thanks
