Phew. I was just reading the downloaded form and the consent section is part of declaration that you accept before submitting application. No seperste form is required this is for set m.

I am still unclear on the passport.

So for my uk national son and myself should i not upload any id documents?

Also with the cohabitation documents. When it says 6 documents do they have to be from 6 different sources or 6 documents in total spread over the period of time

You scan them yourself. However you must also take the docs to the appointment so they can be rescanned if there is an issue with your own scans. You must provide 2 years of cohabitation documents.
