I guess the online form you are referring to is the for you filled out online when you where booking your appointment. When you send an application, you receive acknowledgment letter thereafter and the letter would have you application number. That is the number you need to include when writing your...

Hi,

It is possible to retain your residency since you have been married for 3 years and it's good that you have all his documents.

You just need to apply for retention. But first, you need to have been divorced as you will need to include your divorce certificate in the application.

It does happen sometimes. But its mostly from naturalisation office and not when you are applying for passport.

In my opinion, they only request these when they have a reason to believe that an application may be based on fraud, sham marriage or relationship
