With the Home Office, it's always wise to do what they ask. Fill in Appendix 1 and send them the return envelope, even if it seems like a waste of time to you. If you don't, they're likely to reject your application and then you have to start all over (or appeal—either way it's even more work).

I assume you've already done the Home Office's small survey to check whether or not you are already a UK citizen .
