This is very clearly written in the guidance on the ukba website under their appendix sections.
Your example would be 0 days out of the country, but you do need to declare the times you did go out.

As far as I'm aware, if your wife is on her own visa, she can only apply for ILR by herself, and NOT as your dependant. You can capply on set (O) with your dependant child, but your wife will have to wa til next year to apply for herself.
