Seems like a simple and plain application. If you are under 32 years, you can claim additional points by submitting your Birth Certificate/Driving license as well.

As per requirements mentioned in HSMP Application Form, one is supposed to provide a named contact (employer) who is suppose to verify the earned salaries. This fact can be mention in cover letter. I decided to provide this information on company letter head to make it more official looking. Regards,

I did the same :). I made all 12 wage slips print out fresh and get them sign and stamped at the same time.

However, stamp on my wage slips are "Manager Finance, XYZ Company", with signatures of my finance manager.

If wage slips are on official letter head then sign and seal are optional. Seal will reinforce that the wage slips are original. From my personal experience, the wage slip should show the date for which it is issued.

The signature and official stamp may not bear any date at all.
