Once HSMP is approved, you will need to apply for FLR(HSMP) within 6 months, the HSMP visa is granted for 2 years....so...in your case it will be till 2009....

While applying for FLR(HSMP) include all the dependants in the application (if the dependants are staying in UK) otherwise you need to get their EC done from ur home country...

Not sure whether address & phone no. is mandatory. My letter was accepted, it has address of university but no telephone no.

Have mailed the form. Had got hold of this from in 1st week of DEC from workingintheuk website.
Please get it confirmed from HO if they have modified the form since Dec 2006.

Provided letter from university on university letter head, signed and stamed with following details
date of the award,
full name,
year of passing
and whether the course is taught in english.
