Ditto Olisum, its the date your leave to remain/enter was activated by IND i.e. stamp in your passport. In your case leave to enter as a work permit holder activated on date of entry into UK.

They normally give appointment date on the day starting from when your application is due/valid.(U have to phone in on this day) I.E if its ILR then they accept appointment requests from the 28day that your 5yrs is up.
