Is it necessary to send a cover letter with hsmp(non-jr) to Tier 1 extension applications?

Can anyone tell me what happens if your application is still being considered and your leave to remain expires while waiting,
1) Should you continue working?
2) When your visa is finally approved, what date does it start from, date of approval or date of last expiry?

HSMP to Tier 1

Can anyone tell me if it is appropriate to request an urgent decision at the time of application if I have a continuous job offer? i.e The job I am currently in is on-going.

Well, I think you should re-apply apologizing for not submitting the last 12mths slips and then include it this time around. It shouldnt be much of a problem.
Dont forget to state that ur original copy was not returned to you and add a photocopy of your approval if possible. I wish you the best.

Please can someone tell me if I'm meant to fill in the same amount on both my form and my spouse's form where the amount available for the trip is asked?
