HSMP Application form

I have few queries in filing HSMP Application form. Q30. Amount you earned: £ 6600+ or 6 Lakhs ??? Do I need to mention Indian Salary or GBP? Q31. Please insert the relevant income band from the guidance notes: 40 Pts or ???. I was not clear with guidance notes. Please advice. Regards, Venu.
