u dont have to worry in that matter I guess.

Unless there is a specific reason u r not disclosing to this forum, I think everyone, including urself, gets 2 yrs for the initial approval.

Try faxing a covering letter along with the appropriate page from application (this time making sure you tick the exceptional request).
And followup with the HO over the phone to make sure its been linked to ur file.

Is the salary being paid into your A/c in India?

If so, you should compare yourself with Indian band of earnings.

This is clearly explained in the HSMP guidance notes. Would suggest a good reading of guidance notes -past-earnings section.
