Ofcourse you can apply in advance but if you are planning to apply in person, the time you get your appointment, it will anyway be a month before your visa expiry date.

How much is the difference between your pay slips and the bank statements? I had a difference of 40p in one of my pay slips so I attached my P60 form to support it along with my bank statements.

Please bear in mind that P60 will only cover your earnings during the period Apr'08-Mar'09.

The appointments are booked for next 6 weeks only and everyday the appointments are opened for the next day (ie the day after the sixth week), so keep trying at sharp 8:58-9:00 am and you might get an appointment. Send your application through post in case your visa is expiring in next 6-7 weeks.

You will be fine on both of your questions but make sure the amount is same in the pay slips and bank statemetnts. You can also specify that in your cover letter and highlight the earning's entry in your bank statement to make things easier for the case worker.

I have my own limited company and gave my partner's name (to verify my earnings) who was not contacted.

But I remember case worker saying (at 11:30 am) that checks for my documents haven't come yet (don't know who they contacted and what checks they performed).

You claim points on your gross income, so if this "business miles" a part of your gross income you can show them as a part of your earnings. Your gross salary anyway is coming more than 22K (assuming your monthly gross to be 2185.18).

I am not very sure but this information should be provided from your current visa. So in your case, the answer to D3 should be the date on your HSMP visa i.e the date it was issued to you.
