You don't have to worry. Keep positive, it is due to the back log of applications thats why sometimes the applications take longer.

No the work permit number is a different thing. You don't have to fill this box if you don't have a refernce number. Leave it blank.

Unfortunately it has to be consecutive 12 months in the recent last 15 months period so you have to show January payslip. I hope you will be able to get the required score by including january payslip.
