I would not bother to send approval letter. Remember, it was valid for 6 months only and usefull for your initial application.

About bank statement. Don't worry, it will be case with many of applicant. That;s perfectly fine.

No, there is no such rule. You can enter anytime, within visa validity.

Only problem you need to take care of is having sufficient period left for extension.
