you don't need to send it. I got my extension without sending it.

yes. Dependants have to fill a separate form.

I was planing to apply for extension, but just checking it now have realized that the footer of form for dependent has changed. I have the one with 06/09 and the one available on net is 08/09. You can show the funds for both of you from your account.

You need to maintain a minimum of £1333 in the 3 months before you apply.

Don't worry about rejection. Provide all the required docs and you'll be thru.

Its £6.4 if your application weight is < 1Kg.

No harm if your statements shows August month salary, as long as you mention the earnings period in the cover letter and fill the same in the application form.

I used small sticky labels during my initial HSMP application.
But its better not to do anything with the bank statements.

I just mentioned the bank page numbers in the cover letter this time when I applied for extension.

depends on which application form you are talking about.

If you are applying for extension from inside UK, you have to tick in the boxes.

based on my experience and from what my friends have said, x-ray report is the main thing.. most of the times they don't check the x-ray, but its better to have it in your hand to give them just in case they ask.

I think that shouldn't be a problem as long as you mention the overlapping 3-month period clearly in the cover letter.
