So they need to only sign the printed online form, or I need to ask them separate reference letter on letterhead (from a second referee, who knows professionally).

I hope it should be OK, as I have signed in my ILR A - No, B - No, as discussed earlier, and signed form1 only.
And when she apply, she will put Yes, Yes, as she is financially dependent on me and we will submit a utility bill as a proof of cohabitation.

There are 3 forms, please suggest which forms did you and your wife signed, if you are sharing just the utility bill. Moreover, please suggest did you mention about this in the cover letter, how did it go, will appreciate any answer from you.
