Hi all,

I have contacted ukba and asked them to fix the form. In reply email, I was told that concerned team has been informed and they will be working on fixing the form , no ETA Yet.

Can you please provide the link and page number on which it's said that you should declare FPN ?

Looking from your name looks like you got the Entry Clearance after getting HSMP ? if this is true then you might be able to apply for ILR rather than having to apply for extension. Hi I have exact same dates as you have ! You can apply 28 day before your visa expiry date which in your case will be 20 Nov 2012.
