Could someone please clarify the following SET (O) queries ?

1) I have booked premium appointment and paid the full amount . What do I need to fill in items 5-12 of the payment details sections ? Shall I leave these as blank as I have paid the full amount ?

Is it 6 documents for each source spread over 2 years which will be 18 documents in total or is it 6 documents with a mix of 3 different sources for the 2 years ?

Regarding dependants stay in UK for 2 years, do we need to provide any documentary evidence of stay while applying for ILR ?

I never received acknowledgement letter. However, I have received biometric letter on 30 May and provided same day.

I applied for Tier 1 General extension on April 4 2012. Application received by Home Office on 5 April and fee deducted on 11 May. I have not received any acknowledgement letter nor any reference number.

Is there any way to check with UKBA and when will I receive my biometric enrolment letter?
