to be honest, if you can spare £150 for the English exam at Trinity College, do it mate. It's easy and it will remove your worry about passing the HO requirement for the English language. I did it even if I have the UKNARIC certificate that I used 5 years ago when I was granted Tier 2 Visa General. ...

No cover letter is required but in my case, I had to justify why my family had to arrive after 3 months of our leave granted (after 103 days to be exact), so I made a cover letter to explain (the main reason was about finishing school first before coming to the UK).

I've added my wife and children in my application. Absences for children are not required but I've added it too. No harm of having it anyway.
