When submitting Applicant and sponsor passports visa self scanning which pages need to be scanned and included?

All pages (including cover, back and blank pages), or only photo page and visas/entry+exit stamps?

If the payslips do not show employer’s name then do they also need to be stamped by HR , or do I just need the accompanying letter from the employer confirming they are genuine with no stamps. There is nothing that says they need to be stamped by anybody, only the letter confirming the payslips are...
