Write "not applicable" as the surname.
Just because her first name is used instead of her last name it does not mean that it's now her official surname. She still does not have one.

All questions on the form must be answered, if it does not apply, answer "not applicable".

Use the official birth certificate. They absolutely can request information from another countries and they do if they need to. Even after successful naturalisation they can still investigate alleged fraud. No, it's not against the rules. Your Irish bank account can be used to receive money from any sources, not only from your employer. Another concern is that this income can be investigated by Revenue (all banks in EU periodically report this to the local tax authorities) and this may come back to you...

I might have asked this before but just wanted to be sure that i get a decision after applying for JR and not wasting my time and money. No one here can ensure or guarantee that you will get a decision after going for judicial review and not end up losing money and time. It is up to you to decide.

Send them a letter and ask for more time to fulfill their request. Then contact your gas or electricity providers and ask for your old bills again. They must have them in their system. Don't worry, this is accurate address for Citizenship division of the Department of Justice.

And even if you win, the judge may instruct the Minister to give you a decision without granting the costs as Minister may argue that they would have given you a decision anyway. Better have some savings before going that route.

It's normal. They often request repeated Garda vettings on the same applicant.
Still it's strange that they're asking for a consent.
