As far as I know if your visa validity is more than 6 months from landing date in UK, you need chest x-Ray. In HSMP visa Immigration officer assumes you will be staying in UK for long time and hence it's required.

You can try using www.booking.com for hotel booking. As per their standard policy they don't charge any booking amount but there are some hotels who charges 1 day fee. Read the fine print carefully before booking and you should not have any problem.

I think there are no fixed timelines and varies from case to case. I applied for my EC on 2nd Jan from Mumbai and received stamped passport at my home on 10 Jan 2008.

Typically lead time in Mumbai BHC is 12-18 working days.

I don't think you can calculate your salary in that way. What I understand is Applicants need to show the actual earnings.
