I had permanent residency via EEA Family permit and recently got naturalised about 2 weeks ago. My CEFR Level b1.1 from Trinity college was dated 2016, never needed it to get permament residency though so not sure if that makes a difference.

You an employee of your LTD, just submit the same as a permie would - I just sent my p60s with the application. I have been a contractor for 11 years so thats all the employment details I added - approval came through last week with no questions.

Yes her pay will be taken out as dividends (no NIN/only dividends tax) and her accountant would normally advice her to also be listed as an employee so she can get PAYE cheque every month, though this may be under the NIN threshold.
