You dont have to fill in 3b. Its only for those who are here on HSMP and want to file for extension.
For the visa information in 2D you should enter your work permit details and not the visitors visa or business visa details.

Thats right, you need to use the new form and submit. You will get 3 years extension since you are applying first time on HSMP(Tier 1).

Call the HO and ask them about your problem. I think that now you should make use of Tier 1 application. Since you are not appealing and resubmitting as new form, you should use new application form.
