If you use a separate sheet, I would also compile a list of documents that you are putting in your application in order to formally record its existence. I would also date it, name it, and sign it (the separate sheet)

You have received your Irish citizenship, and your passport, correct? This is now giving you a separate status, which is one of an Irish citizen.
