The 2006 version came into force from Monday 3/04/06, its called HSMP 1(Form version 04/06) valid from 03/04/2006 to 18/06/2006). I used this form as I applied in June 2006. I understand. The UKBA should be able to look at your original form for you, request them to check that.

My absences excluding travel days are coming to: 171 d, plus 17 d of official study leave.
I plan to add detail like Conference name and venue for study leave letter, as it may make the case worker's job easier.
