You should not have to leave the UK. You should be able to switch to the Skilled Worker visa from your student visa within the UK: https://www.gov.uk/skilled-worker-visa/switch-to-this-visa There are new rules being implemented to prevent this switching, but they will not come into force until stud...

If no evidence is required, then no evidence is required. I was not asked for anything either. The simplest answer is that they already have all of the information they need from your original PBS dependent application.

Yes, for the IHS website, you can simply enter today's date and a date of +3 years minus one day. This is just to ensure that you pay the correct IHS fee, which is three years. I applied for my PBS dependent extension just yesterday (my wife is already on ILR now) and I did this same thing.
