I have the same thing with my indian passport. Actually i think its very common on Indian passports. My surname is in given name but didn't find any problems with that yet as i always put my surname (eg: Doe) on my visas. Did you have any problems with empty surname yet?

I used the covering letter and spreadsheet from the sticky threads on this forum. Alo no proof for scoring as I already provided them for tier1 application in 2009. There is a checkbox on SET(O) form for that.

They didn't ask me for employer letter, don't think you need one if you are on tier1.

Cw didn't ask me anything other than the above docs. However my p60s should've proved my employment on WP. I had WP 3 yrs and tier1 2 yrs too.

The date when you received your WP has no relevance here. What is relevant is the date of your entry in to UK as a WP holder (if you applied for entry clearance from outside UK) or the date on which leave to remain was issued (if you applied in-country). I'm planning to do PEO appointment for my ILR SET (O) (WP 2yrs + Tier 1 3 yrs) in Nov 2011. I've got 5 yrs p60 and last 12 months pay slips. Do i still need employer letter from my current employer? From what i understand, employer letter is necessary only for ILR from workpermits?

i applied on 9th jan in-country WP->tier1. found that yesterday, they called my employer for verification. did they called your eemployer as well? Because i am assuming this happens only for WP to tier1 applicants.
