multi entry visitor visa is issued after several visits, not as first time visitor visa.
lets say after 2 successful visits, and third visa could be issued multiple entry, for example.

as soon as your application is checked, your original documents will be returned with a letter informing you to go to GNIB for the stamp 4 for 6 months. Chances are that without this letter GNIB won't give you an extention.

if your application was accepted and a reference number issued, that means that they counted the time on stamp2 as reckonable residence. Now your application will be processed depending on your garda check and the employment, etc... chances are really good, unless you came the attention of the garda...

general immigration can provide them residency on stamp 3 provided that the non visa required parent of irish national can support themselves without being the burden on the state.
c visit visa is not extendable and residency cannot be given to a visa required parent of an irish national

many people who applied in 2009 and 2010 are still waiting. I also know a few that are still waiting longer than that. It's just that when the minister decided to speed up the processing, he did not start from the older files in the queue, but from the fresh applications coming in.... but hopwfully ...

There may be a problem under EUTR due to the fact that the EU citizen has both british and irish citizenship. I think Monife's case was similar and she couldn't go for EUTR, but spouse of Irish national instead....

No, it has to be a comprehensive insurance from ireland. Please note that under the EUTR the comprehensive medical insurance has to cover the applicant, the spouse and the children, if any.
