minors under the age of 16 do not get GNIB numbers, therefore, I would suggest to keep that empty. the section will probably enter your own GNIB number with the application as you are the parent applying on your child's behalf

time spent on business permission does not count towards LTR. only time spent on work permits, work authorisation and working visa count towards LTR. LTR is an administrative scheme, they are allowed to change the rules as they go. But the business permission was never counted for LTR.

Ireland Hopeful, children's applications are processed in a different queue, faster than normal applications. I guess there would be not need any outside agencies. The fee for approval for children is 200e. Not sure how long it takes for the complete process though.

realgunner, on what basis does your father have stamp 4? if your father received irish citizenship, you could get stamp 4 as a dependant of an irish national. you do not qualify for long term residency, if that's what you meant about waiting for 2 years.

HSE, just to let you as you are a doctor, you may have spent some time in the state as a temporary registered doctor, which may not be counted towards your 8 years.
