Do they call individually or couples are called together for interview? Individually. When you book the appointment you will get separate slots. Just received our approval letter! It took us 160 days in total to get it (applied on 28 March). However, we'd requested discretionary for my wife as she had spent more than 90 days in the last year so that was also a factor.

If you carefully read the application form, for naturalisation there is no requirement for the referee to be engaged professionally with the applicant. He needs to have professional standing. Many thanks for your fast reply. We will certainly provide an explanation along with medical records if needed. The guidelines seem a little reassuring as she is at least within the 450 days limit.
