HSMP is closed for new applicants. You can only apply for Tier 1 visa to work as a highly skilled person. So it means "NO". Your explaination in part 8 would be enough to clear any doubts related with returned as invalid.

I think you may submit both evidences together, but claiming period of consecutive three months should be exactly the same for both sources, kind of tricky though. Explain it in cover letter.
