The issue of photograph is becoming more serious with HO. I don't think you can appeal if the case worker has not sent an appeal for AIT-1 to you. I will suggest you make a fresh application ASAP if this is possible.

Please follow the instruction given to you by the AIU. It is necessary you have the photographs certified. You can give this to the solicitor that handled your appeal for you, if you used one during the appeal. whichever way pls make sure you comply by the rule.

You are probably applying for a job with NHS. They are only saying that they are not ready to apply for work permit for any applicant. whoever is applying should have a valid visa that can make him/her work in the UK.
As an HSMP holder you are qualified to apply
