Can anyone point me in the direction of a decent example (or template) of a cover letter that i (UK citizen and sponsor) and my fiance (applicant) could send with our spouse's visa application?

You'd think the ECM would review refusals before they issue the refusal notice. At the very least it would save them time and money in letters and phone calls. Do my documents (passport, birth cert., etc.) need to be certified or notarised? I ask this because having them notarised is more expensive than just having them certfied.
